The role

Do you have what it takes to lead an award winning integrated outreach team for people experiencing homelessness in Southwark, committed to developing new ways of supporting and engaging with people with multiple disadvantage?

This post is responsible to both the SLAM Trust Homelessness Service Lead and Mental Health Service Managers from Adult Social Care. You will lead a small integrated multi-disciplinary team. You will be based within a friendly, creative, collaborative and supportive department at Trust offices in Kennington and also have access to the council offices in Camberwell.

The team offers wrap around, multi-disciplinary support to clients, including statutory social work, nursing, psychiatry and peer navigation and works closely with third sector partners in the borough, including day centres and St Mungo’s. You will act as a bridge between statutory services including health and housing and charitable organisations to ensure a high-quality offer to people with complex needs, who have often not been able to access mainstream services.

The team aims to deliver outreach based, relational care to service users, to work with them to improve quality of life across all Care Act domains and support them across transitions into stable accommodation and reduce the likelihood of return to homelessness.

The role will need to promote safe, innovative and creative approaches to meeting the needs of both service users and carers in line with best practice and quality assurance, alongside performance and budget management, and management oversight of resources and cases

About you:

You will be a registered Social Worker and have experience of managing others within a health, adult social care and/or voluntary sector setting.

This is an exciting role that requires a candidate with excellent skills in communication, organisation and developing partnership working, with proven experience in leadership to help to continue to develop a high-quality service for Southwark residents who are experiencing homelessness, both on the streets and in hostels.

You will have a genuine passion for providing services for people experiencing homelessness and multiple disadvantage and demonstrate a commitment to supporting their recovery with relational care and advocacy.

You will have awareness of current social care legislation and policy including safeguarding adults. You will have understanding of the application of the principles of equality and diversity and implementing it in practice

You will be a practical thinker and have the ability to manage demand while keeping the person at the centre of our strategic vision. We are looking for an innovative and experienced leader with the drive, resilience and creativity to help lead a busy team within a high performing inner London borough.
